HealthCheck360 is a wellness company devoted to improving health and overall well-being. HealthCheck360 provides you with the tools, motivation, and support to assist you along your journey to a healthier you. You will receive a personalized, confidential report of your health status after you complete and submit primary care provider form to myHealthCheck360. HealthCheck360 uses the same confidentiality standards as your doctor.

Wellness Program Overview

HealthCheck360 is partnering with Spirax in 2021 to offer Lifestyle Rewards Points Program to all employees and covered spouses. As a part of this program, participants will be able to participate in wellness challenges and track various aspects of their health (activity, nutrition, sleep, weight, etc.). The site syncs with many fitness tracking devices. Lastly, HealthCheck360 employs health coaches who are available to work telephonically with our remote employees and covered spouses to improve their HealthCheck360 score! Participants can work with one coach throughout the year to allow for a personalized coaching session.

Wellness Premium Reduction

1. Submit a Physician Form by December 1st, 2021.

-Employees receive a 20% premium reduction

-Spouses receive a 10% premium reduction

2. Score a 71 or higher on your health score or improve by 5 from your previous screening.

-Hit the score outcome for an additionial $43.33 premium reduction per employee and per spouse
