Back to Basics Home Medical Visits is a house calls based practice geared towards providing medical care to older adults and persons with mobility and transportation issues who are otherwise unable to access medical care.  Once a month a licensed medical provider visits the patient in their home and conducts a comprehensive medical exam including any necessary labwork, xrays, etc. in order to develop a personalized, patient-centered plan of care.  The provider is able to prescribe any and all medications needed, order home health/hospice services, etc.  Medication reconciliations are performed at each and every visit as well. 

The object of the service is to keep the patient at home, in the comfort of his home for as long as possible without repeat trips to the hospital/emergency room or admission to long term care facilities.  Every effort is made to keep medication costs down and medications to a minimum in order to decrease unwanted side effects and falls. 

The cost of the service is no more than the cost of going to a regular doctor’s office for a scheduled visit.  Insurance companies pay for this service just like a doctor’s office visit, and there is no waiting in a crowded lobby with sick people or getting out in the weather in order to be seen.  The provider comes to you.  In the event that an illness or emergency occurs between visits, an additional visit can be arranged or treatment can be started until the provider can see the patient to avoid hospital trips or endless waiting room time.

The American Geriatric Society recommends that older adults be seen an average of 10-12 times per year in order to maintain their health and wellness.
